Bilingual Education - Essay Example In his autobiography Hunger of Memory, Rodriquez unintentionally portrays himself as an exception to his own beliefs. (Rodriquez 12) This book tracks this Mexican American from the time that he was a "schoolchild until he became a literary scholar and nationally acclaimed memoirist. His memoir also describes his gradual alienation from his cultural roots as his assimilation into mainstream culture deprives him of his native tongue and his connection to his past. The book achieves popularity at a time when multiculturalism is becoming a force in American education." (Burt 2004) In order to make sense of the factors that make up Rodriquez's life, he chooses his love for languages to explain how his persona is formed. Rodriquez surmises that it is better to have used English in school rather than to have had a bilingual education. As he struggles to prove this, he represents himself with a paradox of emotions, thoughts, and beliefs. It is his opinion that it was the different uses of languages and his reaction to them that sets him apart. According to Rodriquez, "It is not possible for any child ever to use his family's language in school. " He thinks that "not to understand this, is to misunderstand the public uses of schooling and to trivialize the nature of intimate life and a family's language." During this time, he also observed that his brother and sister enjoyed a relationship of togetherness, one that left him feeling alone. His sense of alienation continued to grow as he listened to them " communicating among themselves in Spanish." (Rodriquez 1) Rodriquez has been criticized for cutting himself off from his roots. A better argument would be that the languages barriers that were forced upon him caused this gap. Had he been given a bilingual education this would have been different. Instead of living in language compartments at his home and school, his new English language would have blended into that of his native tongue. As his brother and sister did, Rodriquez would have used both languages interchangeably.If one reads Rodriquez's work closely, they sense his great affection for his family. The feelings of shame that he talks about are more general than personal. Nevertheless, it is easier to attack the character of Rodriquez than to discover the bonds that exists between him and his people in spite of their physical and psychological differences. Coming of Age in Somoa Margaret Mead’s â€Å"Coming of Age in Samoa†, which was actually her doctoral dissertation, was compiled in a period of six months starting in 1925. Through it, people were given a look at a society not affected by the problems of 20th century industrial America. She illustrated a picture of a society where love was available for the asking and crime was dealt with by exchanging a few mats. This book helps one to realize the large role played by social environment. One of Mead’s biggest challenges was probably the fact that her fieldwork was done entirely in the Samoan language. In Samoa, few, if any natives spoke English. To get information, Mead spent her time talking to approximately 25 Samoan women. However, she spent much of her focus on two young Samoan women, Fa’apua’a Fa’amu and Fofoa. It is said that one Samoan woman’s life is very much like the next. At the time of her visit to Samoa, Mead, a graduate student was only 23 years old. She was barely older than the girls she interviewed and lovingly called her â€Å"merry companions†. The vision recieved while reading â€Å"Coming of Age in Samoa† is that it is a place of nearly stress free living. The children pass through adolescence without the many pressures put upon teenagers in an industrial America: ...adolescence represented no period of crisis or stress,but was instead an orderly developing of a set of slowly maturing interests and activities (95). According to Mead, families are large, taboos and restrictions are few, and disagreements are settled by the giving of mats. The stresses encountered by American teenagers are unknown to their Samoan counterparts. Mead refers to premarital sex as the â€Å"pastime par excellence† for Samoan youth. She writes that Samoa is a virtual paradise of free love, as the young people from 14 years of age until they are married have nothing on their minds except sex. Of Samoan girls Mead says: She thrusts virtuosity away from her as she thrusts away from her every other sort of responsibility with the invariable comment, â€Å"Laitit a’u† (â€Å"I am but young†). All of her interest is expanded on clandestine sex adventures (33). She explains that growing up can be free, easy and uncomplicated. There are two types of sexual relations other than marriage that are recognized by Samoans. These include love affairs between unmarried young people, and also adultery. Although virginity is not expected in girls, Mead claims that it defiantly adds to their attractiveness. Essentially, having sex with a virgin is much more of a feat for a man than sex with a girl who is not. Marriage in Samoa is regarded as a social and economic arrangement in which relative wealth, rank and the skill of both husband and wife must be considered. In conclusion, Margaret Mead’s dissertation on Samoa is still interesting after 75 years. The customs of Samoans, especially those regarding sex are very interesting to people of other cultures. Sensorial

Maria Montessori described the sensorial materials as the â€Å"key to the universe† Discuss this statement and give examples to support your discussion. â€Å"The senses, being explorers of the world, open the way to knowledge. Our apparatus for educating the senses offers the child a key to guide his explorations of the world, they cast a light upon it which makes visible to him more things in greater detail than he could see in the dark, or uneducated state. â€Å"(1. Montessori Maria, the Absorbent page 190, chapter 17). Montessori believed that sensorial experiences began from birth to six. At this stage, children learn and develop by using their five senses which help in making mental order in their environment. These five senses are visual sense the child learns how to visually discriminate differences between similar objects and differing objects. Second is tactile sense, the child learns through his sense of touch. â€Å"Although the sense of touch is spread throughout the surface of the body, the Exercises given to the children are limited to the tips of the fingers, and particularly, to those of the right hand. (Montessori, Maria (1997) The Discovery of the Child) This allows the child to really focus on what he is feeling, through a concentration of a small part of his body. In the Stereognostic Sense Exercises, the child learns to feel objects and make recognitions based on what he feels. â€Å"When the hand and arm are moved about an object, an impression of movement is added to that touch. Such an impression is attributed to a special, sixth sense, which is called a muscular sense, and which permits many impressions to be stored in a â€Å"muscular memory†, which recalls movements that have been made. In the Baric sense, the child learns to feel the difference of pressure or weight of different objects, this sense is heightened through the use of a blindfold or of closing your eyes . In the Thermic Sense, the child works to refine his sense of temperature. In the Olfactory and Gustatory Sense Exercises, the child is given a key to his smelling and tasting sense. Although not all smells or tastes are given to the child in these Exercises, the child does work to distinguish one smell from another or one taste from another. He can then take these senses, and apply them to other smells or tastes in his environment. In the Auditory Sense Exercises, the child discriminates between different sounds. In doing these different Exercises, the child will refine and make him more sensitive to the sounds in his environment; there are four kinds of sounds human, animal, natural and mechanical sounds. Dr. Montessori felt that this was the ideal period in the child’s life to introduce him the equipment that would sharpen his senses and facilitate his comprehension of the many impressions he receives through them. So that Sensorial lessons enable the child to learn him by using his hands and his mind. We find that Dr. Benjamin Franklin once said â€Å"tell me and I forget. Teach me and remember. Involve me and learn†. In order to serve this purpose; Dr. Maria Montessori introduced a subject called ‘Sensorial', it comes from the word sense or senses. As there are no new experiences for the child to take from the Sensorial work where the materials are specially designed to enable the child to use his senses to explore different attributes of the world†¦ he child is able to concentrate on the refinement of all his senses, from visual to stereognostic. Dr. Montessori based her method of teaching young children considering the fact that a child between two to six years passes through the ‘sensitive period' for the refinement of sense along with the others and they can be helped in the development of the senses while they are in this formative period. â€Å"It is necessary t o begin the education of the senses in the formative period, if we wish to perfect this sense development with the education which is to follow. The education of the senses should be begun methodically in infancy, and should continue during the entire period of instruction which is to prepare the individual for life in society. † (Montessori Maria, internet) Dr. Montessori describe Sensorial education as â€Å"the key to the universe† because it revealing to the children, a deeper knowledge about the outside world. All of the material is aesthetically pleasing. They attract the child’s attention to the objects and allow the child to manipulate the materials with easy way. The material must be complete. This allows the child who is working with the material to finish through the whole piece of work without having to stop and find a missing piece. All of the material is limited. The first use of the term limited refers to the fact that there is only one of each material in the environment. This calls for other students to build on their patience. The second use of the word limited is in reference to the idea that not all of one quality or piece of information is given to the child. This child is not given every color in the world, but only a select few. This gives the child the keys to the information so it peaks his curiosity and leads him to learn more out of his own interest. Most importantly, all of the material could be called â€Å"materialized abstractions†. This means that though Montessori’s Sensorial materials, abstract concepts are made into concrete materials. â€Å"The sensorial materials comprise a series of objects which are grouped together according to some physical quality which they have, such as colour, shape, size, sound, texture, weight, temperature, and so forth. Every single group of objects represents the same quality but in different degrees; there is consequently a regular gradual distinction between the various objects and, when this is possible, one that is mathematically fixed. Every series of objects is graded so that there is a maximum and a minimum, which determines its limits, or which, more properly, are fixed by the use which a child makes of them† (Montessori Maria, The Discovery of Childhood, Page. 100 chapter 6). Dr. Montessori believed in the same ideal and with her development of Sensorial teaching brought a new concept to teaching the world throughout her Sensorial materials. So that the sensorial materials in the Montessori classroom enable the child to become aware of details by revealing to him strongly contrasting sensations like black and white and progressing to various gradations of this sensation such as; the many different shades of red in the colour tablets as an example, facilitate his knowledge of colours and eventually his understanding of the abstraction of a certain colour and finally the abstraction of color itself. The sensorial equipment, worked on systematically, builds a rooted and comprehensive foundation for the child's intellectual future. This is why Montessori has appropriately named the sensorial materials as â€Å"the key to the universe†. We find that the sensorial materials related with the environment around the child, he try to discover on his own how to make things he see in the environment, he often want to make his own books and do so with tools of ruler and stapler, make his own constructive triangles, or geometric solids. He often explores different ways of making the same end product – e. g. making a cylinder with paper, and then trying with clay. The other areas of the curriculum for the children of this age are related with the ‘sensorial’ materials such as mathematics, language and culture. The sensorial materials respond to the way that the child learns at this age through the senses rather than the intellect. There are materials for the refinement of each sense, with each activity isolating one particular quality, for example; color, size, sound, taste or weight. We can take a pink tower as an example, it made up of ten pink cubes of varying sizes. In 3 year-old the child constructs a tower with the largest cube on the bottom and the smallest on top. This material isolates the concept of size. The cubes are all the same colour and texture; the only difference is their size. Other materials isolate different concepts: colour tablets for colour, geometry materials for form and so on. As the child's exploration continues, the materials interrelate and build upon each other. Later, in the primary years, new aspects of some materials unfold. When studying volume, for example, the child may return to the pink tower and discover that its cubes progress incrementally from one cubic centimeter to one cubic decimetre. At the pre-school age when the child is use the sensory information, these materials help the child to order and make sense of his world and heighten his perception and wonder of it. Through working with the different sensorial materials the child has refined his discrimination of size to the point where he wants to know how much one object is bigger than other one. The mathematic materials flow naturally from here. When a child reaches this point, he needs to introduce to concrete representations of mathematical concepts and given language to describe these, for example: large, long, thick, heavy. They are required to sort, pair, grade and sequence using all of their senses – visual, tactile, auditory, gustatory and olfactory. Through these activities they develop the ability to solve mathematical problems for example: decimal system, geometry and algebra –by compare, contrast, and make judgments. The same applies with language. The subtle preparation the child has been given in this environment such as songs, stories, poems, or the control over the movement of the hand through knobbed cylinders and geometric cabinet, they allow the child from 4 and 5 year olds to effortlessly start to write and read. Montessori education has been using a set of ‘sandpaper letters’ individual boards with the primary symbol for each of the 26 letters as the sounds in the English language. Three year-old children see and feel these symbols and make the corresponding sound, bsorbing the combination of sound and symbol through three different senses (auditory-visual and tactile sense. Finally, the cultural materials bring to the child his world and the animals, plants and people within it. Like everything offered to the child at this age, the materials are sensory-based and are introduced to the child in an orderly way; first the world, then the plants; the child is introduced to the botanical classification in plants and their parts through classified cards, and an experience of how leaves can be classified by their shape through the leaf cabinet. Through these, the child is given keys to enhance his exploration of the outdoor environment and garden, and ultimately the world. The child starts to notice shapes of leaves as he walks in the park, and this deepens his appreciation of his environment. The sensorial materials also are the extension for the world of animals, then mammals, birds, amphibians, reptiles and fish; in the same way as with the world of plants, the child is given presentations of animal classification through the classified cards. Through his exploration he finds the keys to understanding the characteristics of each family and this is applied to his locality. The sensorial materials extend to geography: The children discover how our world can be divided into land and water, and how these two can further be classified by their shape – land into islands, peninsulas, isthmuses and capes; water into lake, gulf, strait and bay. These offer a key to the young mind so attuned to order and pattern, and children start to appreciate even the lakes and islands they discover in their local parks. They also explore how our world is divided into continents, and how each continent is divided into countries, and towns and so on. In this way they get an appreciation of where their ‘place’ is in the world. The education of senses makes men observers. The child who has worked with the sensorial materials has not only acquired a greater skill in the use of senses but also guides his exploration of the outside world. The aim of sense training is not only that a child shall know the colours forms and textures but also that he refines his sense through an exercise of attention and through comparison. The Sensorial Materials have been given many names: materialized abstractions, key to universe, path to culture. The goal of sensorial is to aid a child refine his senses so they can learn more from his environment and grow spiritually and physically. This is done through manipulation with carefully designed materials and direct experience with the world around them. The Sensorial technique is a multi-faceted method of learning. Through the use of singular quality focus activities the child’s senses are awakened. The sensory revelation that is experienced by the child during this period leads to a greater intellectual capability. The sensorial practice sets the groundwork for further intellectual growth. The crucial roles the senses have in education are illustrated through the connection between the various sensorial experiences the child has and all of the activities in the Sensorial environment. Because of Montessori’s focus on sensorial exploration and the Sensitive Periods for crucial brain development, there may be no computers for children’s use in the Children’s House environment. However, through the Montessori materials the children are building strong foundations to be able to learn to use this technology when appropriate. The Power of a Mother’s Love In the story â€Å"Red Mocassins† by Susan Power, she tries to get her readers to feel a certain way about specific characters in her story. The story pushes me to be annoyed with Joyce, and genuinely like Anna. Power portrays Joyce to be lazy, inconsiderate, selfish and, most importantly, a bad mother. The complete opposite of this is her cousin, Anna, who is loving, generous and nurturing. Several places in the text demonstrate the qualities of each of these two women. Because of this evidence in the text, it is obvious that this author wants me not to like Joyce, and feel compassion towards Anna. Power makes Joyce look like a bad mother right from the beginning of the story. Joyce is portrayed not to be â€Å"mother material† because Anna, her cousin, is the one working on Bernardine’s first Sioux costume. Power says in the text that â€Å"Ordinarily a mother would do this, but Dina’s was the next thing to useless. Joyce Blue Kettle had never gotten close enough to a needle to stick herself, let alone sew a costume† (409). A reader already looks at Joyce in a negative way because she should have been the one fulfilling this important tradition for her only daughter. Joyce is obviously jealous of her cousin Anna. Anna recalls stories of their childhood and how obnoxious Joyce was towards her. Joyce’s husband, Clifford, loved Anna, not Joyce, but Joyce soon changed that. Clifford bought Anna many little gifts when they were children. One time Clifford brought Anna a glass eye, and â€Å"It was too much for Joyce. Bolsa Chica Fieldtrip essays As we arrive at the Bolsa Chica Ecological Reserve I notice that this day is particularly warm and sunny. In the distance a significant haze is visible, but there arent many clouds. Its 10:10 AM and weve just been handed our binoculars and data collection sheets. I am standing facing west to avoid looking into the sun, and off to my right I notice quite a few pigeons, which Travis informs us dont really count since they were introduced into the area by people. The air temperature is reported to be about 72 degrees Fahrenheit. The wind seems unusually strong in this area, but the weather reported a wind out of the south east of 10 to 15 miles per hour. My partner for the data collection today is Mariah. If I stick with her today I am bound to learn a few new bird names. As we begin our walk around the Ecological Reserve we stop first on the footbridge about ten feet from where we began. As I peer down into the water below I see a lot of shells littered about. Travis tells us that what we are seeing are razor clams, muscles, sponges, algae, and a few stingrays. I was surprised to find out that this is a hatchery for lots of small fish. I look around as he tells us about the fish, and I see nearly a hundred birds in the distance, and now I realize why they stick around this place. Theres a lot of food for them here. I also see some snails as we begin the rest of our walk. I am excited to see birds at this point and I keep noticing pigeons around the bridge hoping that they are some exotic bird that I can write down, but that will have to wait. I dont have to wait long though. Almost immediately I see my first feeding bird, a Caspian Tern I am told. Its scientific name is Sterna caspia. We watch the bird fly around and then into an Arial plunge dive. It appears to be about 100 feet from the waters edge and it is diving in the Inner Bay area. I cant follow it very well wi...

Rheumatoid Arthritis Treatment Methods INTRODUCTION Autoimmunity is the inability of an organism in recognizing its own parts as  self, which triggers an abnormal immune response against its own cells or tissues. Due to such a response, autoimmune diseases occur. Autoimmune diseases are broadly divided into the following: Systemic autoimmune diseases: The symptoms and damage occurs throughout the body, i.e the antigen is not tissue-specific. Localized autoimmune diseases: The damage is localized, i.e., the antigen is tissue specific. Rheumatoid arthritis (RA) is a chronic  inflammatory, systemic autoimmune disorderthat may affect many tissues and organs, but mainly attacks the flexible (synovial) joints. 75% of the reported cases of RA occur in women, especially at age: 30 and 40 years and between 50 and 60 years (Bach, 1982). It can be a disabling and  painful  condition, which can lead to consequential loss of mobility and function if not treated adequately. But it is still unclear whether T-cells primarily respond to a microbial antigen, or a self-constituent antigen (Chiniet al., 2002). Lesions developed in RA appear to be involved in both cell-mediated and humoral responses. Prior research work focused on identifying the cells present in the affected synovium, and has been concluded that CD4+ T lymphocytes, active B lymphocytes, and plasma cells, combined with well-formed lymphoid follicles having germinal centers (in more serious cases), are present in the synovium of the patients (Abbas et al., 1994). Major cells present in the synovial filtrate of patients are T-cells, and a partial therapeutic effect was observed due to depletion of T-cells in these patients (Berneret al., 2000). Present understanding of RA conveys that TH1 cells which are specific for a particular antigen (which hasn’t been identified yet) are present in the joints of the patients. Pathogenesis and symptoms Fig1.1: Release of cytokines due to T-cell activation causing an inflamed synovium and pannus. Cytokines released in the synovium are Interleukin(IL)-1, IL-8, Interferon-gamma (INF-gamma), and Tumour Necrosis Factor-alpha(TNF-alpha). But the clinically important ones are IL-1 and TNF-alpha. These cytokines stimulate increase in collaginases, IL-6, chemokines, nitric oxide and Cyclo-oxigenase-2(COX-2) production. The combined action of these and a few others like IL-2 and IL-4 lead to the pathogenesis of the disease. Other than antigens, RA also involves antibodies- this is most likely due to formation of immune complexes. The auto-antibodies produced are called ‘rheumatoid factors’ and are specific to the Fc region of IgG. This rheumatoid factor is an IgM antibody and hence the immune complex consists of IgG-IgM which cause the damage (Janeway et al., 2001). Symptoms include stiffness, pain, swelling, and erythema, joints become tender, swollen, and warm. As it progresses, multiple joints would be affected (polyarthritis). The clinical manifestations are collection of polymorphonuclear leukocytes and macrophages at the inflamed region, cartilage damage, and, destruction of the joint Synovitis (inflammation of the synovium) can lead to  tethering  of tissue, lack of movement and erosion of the surface leading to deformity and loss of function. Systemic complexities are damage to various other organs like lungs, myocardia, pericardia pleura, eyes, and Central Nervous System as a result of inflammatory reactions (fig-1.1). Many agents are now available to treat RA, and many of them are monoclonal antibodies. Several new monoclonal antibodies are currently under development and hopefully will be available as other alternatives The focus of this paper is to state the latest therapeutic monoclonal antibodies being used for RA treatment, to state their merits and demerits and whether they are better than normal drugs/medication. Monoclonal antibodies with different mechanisms of action and route of administration are discussed and whether they pose as good therapeutic agents with an acceptable safety profile. 2. REVIEW OF LITERATURE 2.1  Treatment of Rhematoid Arthritis Some of the treatments for RA are: Anti-TNF-alphaTreatments Medications Therapeutic monoclonal antibody treatment Earlier it was believed that if one cytokine signal cascade was blocked then another cytokine would takeover. It was hypothesized that IL-1 caused the cartilage and bone damage.Through a study(at Kennedy Institute of Rheumatology), it was found that IL-1 bioactivity had stopped due to TNF-alpha blocking. This led to the development of anti-TNF-alpha treatments like- cyotokine antibodies and soluble-receptor antagonists (Feldmann et al., 1999). There are mainly four types of medications being used- Disease-modifying anti-rheumatic drugs (DMARDs), Non-steroidal anti-inflammatory drugs (NSAIDs), immuno-suppressants, and corticosteroids (glucocorticoids).(Rang et al.,1995). DMARDs include pencillamines (Cuprimine ®, Depen ®), gold compounds (Myochrysine ®, Ridaura ®) and chloroquine (Plaquenil ®). Although their mechanism of action is not properly understood, they have a very significant effect on RA patients. NSAIDs like asprin, ibuprofen (Advil  ®), ketoprofen (Orudis ®), naproxen (Naprosyn ®), etc.. have a variety of effects like anti-inflammatory, anti-pyretic, and analgesic effects.Immuno-suppressants like cyclosporine and cytotoxic agents like azathioprine suppress both humoral and cell-mediated immunity. Corticosteroids like prednisone, hydrocortisone, etc., have immunosuppressive and anti-inflammatory effects (Rang et al.,1995). 2.1.1 Therapeutic monoclonal antibodies: Each B-cell synthesizes only one kind of antibody and every organism has various populations of B-cells which secrete various antibodies specific to various antigens being recognized. But in order to turn this function into a helpful tool, we need huge amounts of a same antibody. So we need to culture a B-cell population originating from the same ancestral B-cell and hence obtaining the same kind of antibodies.Such a population of cells are called ‘monoclonal’ and the antibodies are‘monoclonal antibodies’(mAbs).Such monoclonal antibodies used for various therapeutic purposes are referred to as ‘therapeutic monoclonal antibodies’. If a specific antibody of a B-lymphocyte is needed, that B-cell should be secreted in an organism. So the antigen for which the antibody is needed is injected into a mouse and it secretes the B-cell antibodies against the antigen. Such B-cells are isolated from the spleen and fused with myeloma cells (using Poly Ethylene glycol, or electroporation), forming hybridomas. To selectively isolate the hybridoma cells, they are grown in HAT(Hypoxanthine AminopterinThyamidine) medium. (Fig 2.1.1) The cancer cells are HGPRT- and the B-cells are HGPRT+(HGPRT is an enzyme Hypoxanthine-guanine phosphoribosyltransferase which helps in the synthesis of nucleotides from hypoxanthine). Therefore, all the unfused myeloma cells die in the HAT medium and all the unfused B-cells can’t divide for long and only the hybridoma cells survive indefinitely. It is from these cells that the antigen-specific antibodies are labelled and isolated using radioactivity or immunofluorescence. Fig 2.1.1- A summary of the process of monoclonal antibody production. Types Till date there are four types of therapeutic mAbs- Murine Chimeric Humanized Human Murine antibodies (suffix omab)refer tomAbs made from any mammal of the family muridae (like mouse, rat). These mAbs were produced using hybridoma technology and were analogous to murine antibodies. But these were not a success because immune complexes formed due to which they only had a short half-life in vivo and caused cytotoxicity resulting in allergies and anaphylactic shocks .Hence these were replaced by chimeric and humanizedmAbs.Chimeric mAbs (suffix ximab) has variable regions of murine and constant region of humans fused together (they are 65% human). This decreases immunogenicity. Humanized antibodies (suffix –zumab) are made by grafting murine hyper-variable region onto the amino acid region of the human Abs (they are 85% human). However, these antibodies lacked the specificity of their parent murine mAbsso, affinity was increased by introducing mutations in the CDR (complementarity determining region). Phage display libraries or transgenic mice are use to produce human mAbs (suffix –umab). In this the murine genome is injected with the human immunoglobulin genes due to which it becomes transgenic. This mouse is injected with the desired antigen to yield the subsequent mAbs(Hudson PJ, Souriau C., 2003). 2.1.1.1 Monoclonal Antibodies Directed Against TNF-ÃŽ ± TNF-ÃŽ ± is a key mediator of the inflammation-induced joint damage that is a hallmark of this disease. Monoclonal antibodies to TNF bind soluble and transmembrane TNF, thereby down-regulating TNF-induced immune responses including adhesion molecule expression, cytokine production, matrix metalloproteinase production, neutrophil activities, dendritic cell function and osteoclast differentiaion. (Blumi S, Jet al.,2012) Monoclonal antibodies to TNF, except for certolizumab have the ability to lyse TNF-expressing cells in the presence of complement. (Kukar M, et al.,2009). Currently there are four mAbs approved for the treatment of RA Infliximab This is a chimeric IgG1 mAb and has human constant region with murine variable regions.(Perdriger A., 2009). This is best used with methotextrate(MTX) and is available only in the intravenus form. In 2001, it was approved by the FDA in combination with MTX to treat moderate to severe RA. Effectiveness and improvement in the disease compared to placebo was shown in multiple, randomized trials(Elliott MJ, et al., 1993) ,( Lipsky PE, et al., 2000), (MainiRN, et al., 2004). It was proven to be effective in early stages of the disease (et al., 2004).Given that influximab is comprised of a significant proportion of murine protein it was anticipated that patients would develop antichimeric antibodies that could impair the efficacy and increase the risk of infusion reactions. The combination of infliximab and MTX results in a substantial reduction in antichimeric antibody and increased serum infliximab levels.. Adalimumab Adalimumab is a human recombinant IgG1 mAb that has no murine component and is produced by phage display technology. It was FDA/EMA approved in, or soon after, 2002 for the treatment of moderate-to-severe RA as monotherapy or in combination with disease-modifying antirheumatic drugs (DMARDs). It is available in the subcutaneous form at a dose of 40 mg every 2 weeks. Despite adalimumab being a fully human antibody, anti-adalimumab antibodies have been detected in a significant number of patients(Vincent FB, Morland EF, Murphy Ket al.2013). Adalimumab responses and long-term sustainability may be reduced by anti-adalimumab antibodies, but adalimumab generally has good sustainability similar to that of etanercept and generally better than infliximab. Golimumab Golimumab is a fully human IgG1 anti-TNF-ÃŽ ± antibody that was generated and affinity matured in an in vivo system. It is very similar in structure to infliximab without the mouse protein. It was approved by the FDA/EMA in or soon after 2009 for the treatment of moderate-to-severe RA in combination with MTX(Kremer J, et al,2010). Certolizumab Certolizumabpegol is a humanized Fab fragment (Fc free) fused to a 40-kd polyethylene glycol (PEG) moiety. It was FDA/EMA approved in 2009 for the treatment of moderate-to-severe RA as monotherapy or in combination with MTX. It is available in the subcutaneous form at a dose of 400 mg at 0, 2 and 4 weeks, then every 2 weeks or 400 mg every 4 weeks. 2.1.1.2 Antibodies against B Cells B cells are critical to the pathogenesis of RA. Mature B cells may evolve into antibody producing plasma cells. Although the precise role of B-cell-producing autoantibodies in RA remains unclear, B cell and plasma cell infiltration into synovium has consistently been found.In addition to their role as precursors to antibody producing plasma cells, B cells may function as antigen-presenting cells and may also produce inflammatory cytokines and co-stimulatory molecules important for T-cell function.(TngYKO,et al., 2007). Rituximab As rituximab is a B-cell-depleting agent, chimeric/IgG1 monoclonal antibody which binds to the CD20 cell surface marker found on several maturation stages of B lymphocytes. It gained FDA/EMA approval in 2006 for the treatment of moderate-to-severe RA in combination with MTX in patients with inadequate response to anti-TNF. Rituximab is given via the intravenous route at a dose of 1000 mg for two doses 2 weeks apart for each cycle. The first study evaluated rituximab in RA was reported by Edwardset al., 2004.Four treatment groups consisting of MTX monotherapy, rituximab monotherapy, rituximab plus cyclophosphamide and rituximab plus MTX were compared, and all rituximab groups had a better ACR20 response compared with MTX monotherapy, with a comparable safety profile. 2.1.1.3 Antibodies That Interfere With IL-6 Function IL-6 is a cytokine produced by immunologically important cells that has an important role in T-cell activation and immunoglobulin secretion.It also stimulates synovial fibroblast differentiation and osteoclast activation.Dysregulation of IL-6 is also, in part, responsible for many of the generalized systemic effects of RA, including anemia of chronic disease as well as the acute phase reactants seen in this disease(Tanaka.Tet al. , 2010). Tocilizumab Previously called MRA, tocilizumab is a humanized/IgG1 mAb directed against IL-6 receptor in its soluble and transmembrane form. It was approved by the FDA/EMA in early 2010, or slightly before, for the treatment of moderate-to-severe RA in patients with an inadequate response to DMARDs and/or anti-TNF. A subcutaneous form of tocilizumab is currently under study. It is also indicated in patients with anemia of chronic disease since it dramatically increases hemoglobin as a consequence of reduction in hepcidin – the protein that inhibits iron utilization in RA. 2.1.1.4 Antibodies That Interfere With IL-1 Function IL-1 is produced by many cell types in response to myriad inflammatory stimuli and mediates multiple immunologic and inflammatory pathways. In patients with RA, the levels of naturally produced IL-1 receptor antagonist in the synovium is thought to be insufficient to counteract the increased levels of IL-1 produced in this disease (Arend WP., 2002) Anakinra Anakinra is the recombinant form of a human receptor antagonist (IL-1ra), and was approved by the FDA/EMA in, or slightly after, 2002 at a daily dose of 100 mg subcutaneously for moderate-to-severe RA that has been unresponsive to initial disease DMARD therapy. It has been studied in RA in several trials. (Fleischmann RM.et al,  2003) 2.1.1.5 Safety Infections Infections are the most common adverse event associated with the use of all biologics. Infection risks with anakinra and tocilizumab are probably similar to the TNF inhibitors, with rituximab perhaps having slightly less risk.Infection risk may be higher with the use of increased doses of infliximab and anakinra.Respiratory tract infections are most commonly reported. The risk of granulomatous infections, such as tuberculosis, is also increased in patients using monoclonal antibody TNF inhibitors. Malignancies The use of TNF inhibitors in patients with RA has not been associated with an increased risk of solid cancers, with the exception of cutaneous malignancies.. Malignancies have been reported with anakinra, tocilizumab and rituximab, but the risk does not seem higher than predicted in RA patients.(Ding T et al., 2010)Longer-term follow-up is required to more clearly understand the risk of malignancies with these drugs. Demyelinating diseases Symptoms of demyelinating neurologic dysfunction have been associated with TNF inhibitors. Resolution of these symptoms with drug withdrawal is common. TNF inhibitors should be withdrawn immediately if neurologic symptoms occur with use, and probably should be avoided in patients with pre-existing demylelinating symptoms(Ding T, Ledinghamet al., 2010).   3. DISCUSSION AND CONCLUSION 3.1 Drugs vs mAbs Just as monoclonal antibodies have various side effects the medication used for RA also have various complications There have been many news reports about how pain-killing drugs known as COX-2 blockers increase heart attack and stroke risks. Additional studies suggested that older non- steroidal anti-inflammatory drugs (NSAIDs) like ibuprofen could also elevate heart risks.One of the culprits: methotrexate. It’s the most commonly prescribed disease-modifying anti-rheumatic drug, orDMARD, for rheumatoid arthritis. It’s also responsible for hair loss in about 1 to 3 percent of people. The hair loss happens because methotrexate is doing what it’s supposed to do – stop cells from growing, including cells causing inflammation and, unfortunately, hair follicles. Folic acid, which is commonly prescribed with methotrexate to mitigate some of its side effects, is a synthetic form of folate, a B-complex vitamin. It can help keep hair healthy, but it has not been fou nd to promote hair growth.Generally as a drug-related side effect, the hair loss is not drastic and the hair does not fall out in patches. And it usually grows back once the patients stop taking the drug.So as of now, most of thesuccesful treatments for RA using monoclonal antibodies are in combination with methotrexate. 3.2 Conclusion and future prospects Monoclonal antibodies with different mechanisms of action and route of administration are highly effective therapeutic agents in the treatment of RA with an acceptable safety profile. Choosing the appropriate treatment is a complex decision that is affected by clinical data, physician and patient preference, and payers. Almost undoubtedly, these types of agents will continue to be important agents in the rheumatologists armamentarium. How to use these agents more selectively, particularly regarding which agents are best for which patients, hopefully will be better established in the future with new biomarkers. Prediction as to what agent to use in the right patient at the right time is clearly a research priority. Monoclonal antibodies as new agents are expensive, and the cost/benefit analysis justifying their use is also critical to practitioners. . Many aspects regarding the efficacy and safety of the supposedly cheaper biologics need to be evaluated before they are available for w idespread use, but their availability and the emergence of new agents in the future may substantially change the RA treatment landscape. Although current therapies can reduce the signs and symptoms of RA for many patients, the quest for a cure (or a more complete blockade of the structural damage) in RA is still ongoing and will need treatment approaches, which are not exclusively confined to blocking a particular cytokine, receptor, or auto-reactive B or T cell involved in disease progression. To this end exciting treatment alternatives and drug targets are on the horizon that may become available to patients in the future.
